Not all headaches are migraines. Migraines are a specific headache with specific treatments. I often tell a patient that they Own their Migraine. It is theirs and no two are alike. Every ER doctor has their own favorite treatment for migraines which evolves over the years. My own particular favorite cure is an IV injection of Magnesium with Zofran. I have been using it for over 25 years and have great success with it.
Narcotics do not help Migraines. They cause a worse rebound headache and as such we do not use them. However, If a patient has had experience with a particular treatment that has worked in the past, we are more tan willing to consider it. There is a Regan Protocol or Toradol and Benadryl which has been shown to work on many patients. Others take a simple shot of Toradol and they are ready to go. Again, each migraine is very individualistic to the person.
That being said, not all headaches are Migraines. Stress headaches are the number two reason for debilitating headaches; these respond to a medication called Butalbital with a shot of steroids and a muscle relaxer. But once more, each patient who I have had the headache usually knows what works to fix it.
Lastly, but certainly not least are Sinus Headaches; These result from infection and pressure in the sinuses and need antibiotics and steroids. But sinus headaches almost always have a tension component which responds with the Butalbital as part of the treatment. The point is that not all headaches are the same, but we know the different causes and are open minded to find a solution for your headache. We recognize they are not trivial, they upset your life and make functioning difficult at best.
